MISSION
AFAHO provides health, social, and educational services and programs to the African and Caribbean immigrant and refugee communities in the greater Philadelphia area. These services include a variety of health education, screenings, and linkage to care, case management, adult education, youth activities, and psychosocial wellness programs which are available to the community regardless of age, nationality, language ability, or legal status.

JOB SUMMARY:

SUMMARY OF POSITION

AFAHO’s Crisis Case Manager & Navigator is a key member of the organization’s Health and Social Services team, providing trauma-informed case management, crisis intervention, linkage to appropriate services, follow-up, and other culturally & linguistically appropriate support services for African and Caribbean Immigrant and Refugee (ACIR) clients facing crisis or other significant issues.

This role also supports health and social service navigation, including recruitment and outreach for AFAHO’s health education, wellness, and social support programs. The ideal candidate is a trained social worker or human services professional with a lived or deep cultural understanding of ACIR communities. This person will also be familiar with the ACIR communities in greater Philadelphia and will be comfortable conducting outreach and engagement.

Role & Responsibilities:

Crisis Case Management, Advocacy and Navigation (60%)

  • Provide trauma-informed case management for ACIR individuals in crisis (sexual assault, intimate partner violence, housing instability, safety risks, mental health needs, etc.)
  • Conduct strengths-based assessments, safety planning, and care plans
  • Provide crisis intervention and emotional support
  • Facilitate access to medical, legal, behavioral health, and social services
  • Advocate for survivor self-determination, dignity, confidentiality, and choice
  • Provide forensic exam, hospital, court, immigration/legal, and law-enforcement accompaniment as needed
  • Assist with emergency transportation and basic needs resources
  • Participate in case reviews and multidisciplinary collaboration
  • Maintain accurate and confidential case notes and compliance documentation
  • Support Monitoring, Evaluation & Learning (MEL) and SSM assessments
  • Support referrals to internal and external programs
  • Assist with applications for services (Medicaid, SNAP, ID, financial assistance, etc.)

Outreach, Engagement, & Recruitment (40%)

  • Conduct outreach and community engagement activities in the ACIR communities of Philadelphia and surrounding areas to inform about AFAHO’s services
  • Recruit individuals for AFAHO’s health and social service programs (maternal health, wellness programs, youth, health screening days, elders, Hep B/HIV services, etc.)
  • Support community events, workshops, and resource navigation days
  • Distribute culturally relevant materials in ACIR languages via digital and in-person platforms
  • Maintain strong relationships with faith institutions, community groups, and leaders
  • Develop new and maintain existing relationships with external partners to facilitate access for clients and expand AFAHO’s trusted network of care
  • Track service delivery and outreach activities in program systems. Participate in ongoing professional development training and reflective supervision

Qualifications:

  • Bachelor’s degree in social work, psychology, public health, counseling, human services or related field required.
  • A minimum of 2 years of experience in crisis intervention or case management required.
  • Understanding of trauma-informed, survivor-centered, and culturally responsive practice
  • At least 2 years’ experience working with immigrant/refugee populations required
  • Proficiency in French or an African language preferred.
  • Comfortable conducting community engagement
  • Ability to document activities efficiently and promptly
  • Strong knowledge of local health, social, housing, benefits, and other systems
  • Flexibility and adaptability to handle and prioritize multiple tasks effectively.
  • Advanced interpersonal skills to interact with diverse coworkers effectively, professionally, and compassionately.
  • Excellent organizational and planning skills.
  • Strong commitment to supporting the collective efforts of the team in accomplishing program objectives and goals.
  • Excellent self-direction and the ability to take responsibility and drive responsibilities through to completion.
  • Strong verbal and written communication skills.
  • Excellent problem-solving abilities.
  • Previous Database Management desirable.
  • Must be able to work some evenings and weekends.
  • Advanced Computer Skills Preferred
